Suffering from an advanced chronic and/or terminal illness isn’t easy for patients or their loved ones. Jamaica Hospital’s Palliative Care Services acknowledges this difficulty and is committed to helping its patients ease their pain and discomfort and deal with the emotional distress commonly associated with advanced illnesses.
Our Palliative Care team, which consists of attending physicians, palliative care fellows, and a licensed clinical social worker, works in close collaboration with the following departments: pastoral care, pharmacy, medicine, surgery, dietary, rehabilitation, volunteer services, anesthesia, and social work.
We provide a variety of medical, emotional, social, and spiritual care services, including:
- Pain and symptom management
- Psychosocial and emotional support
- Spiritual support and counseling
- Grief and bereavement counseling
- Patient and families meetings to discuss prognosis, goals of care, and advanced directives.
Specifically, our physicians assess patients for pain and symptoms and provide appropriate recommendations and follow-up; make recommendations for treatment options and discharge based on patients’ preferences, goals of care, and prognosis; discuss with patients and families information about the patients’ illness and consider goals of care and advanced directives; and provide education to the nursing and medical staff about palliative care.
Our clinical social workers provide psychosocial support to help patients and their families make difficult decisions in regards to their health care; facilitate communication with family members, focusing on patient’s wishes; conduct follow-up for patients who are discharged home, transferred to a rehabilitation facility, skilled nursing home or inpatient hospice setting; provide information and referral to community resources; and make bereavement calls or visits.
The Palliative Care Service sees all new consults within 24 hours and the team rounds together daily. A member of the palliative care team is on call 24 hours a day, seven days a week.
