What is Care Management?
The Care Management Programs at Jamaica Hospital Medical Center (JHMC) are designed to assist people who need frequent medical attention. We work closely with primary care physicians (PCP), patients, families and health care providers to support care in a patient-centered, community-based way. All of our chronic care management programs strongly support shared decision making with the patient and family, giving you a voice in your own health care.
