What is Care Management?

The Care Management Programs at Jamaica Hospital Medical Center are designed to support people by coordinating their care and connecting them to services that best support their health and daily lives. We work closely with primary care physicians (PCP), patients, families, and health care providers to support care in a patient-centered, community-based way. All of our care management programs strongly support shared decision making with the patient and family, giving you a voice in your own health care.

Our Programs

Following these guiding principles, our Care Management department has a variety of programs to provide and support the highest standard of care for our patients. All of our programs are at no cost to the patient.

  • Care Transitions
  • Co-Managed Care (Behavioral/Mental Health and Medical)
  • Complex Case Management
  • EMPOWER
  • Perioperative Surgical Home and TEAM (Joint Replacements, Colon Surgeries, Spinal Fusion Surgeries, and Surgical Hip Fracture Repairs)
  • Oncology (in partnership with Memorial Sloan Kettering)

Our goal is to maximize your physical and mental health, improve the health of our community, and to reduce and avoid frequent and unnecessary hospitalizations.   We strive to improve your quality of life and connect you with the support services you need.
