Jamaica Hospital Medical Records
Welcome to Jamaica Hospital Medical Center’s medical records portal.
For your convenience, we have provided several options to request a copy of your medical records. A copy can be requested online, at our Medical Records Department or by mail.
However, because medical records are protected, there is an established authorization process in place. Read below to learn more about how we protect your privacy and how you can request your records.
Confidentiality of Your Jamaica Hospital Medical Records
Maintaining the privacy of all personal or medical information in your health record is your right as a patient. For this reason, we will not release records (With the exception of court orders) to any patient, unless the patient, his or her legal guardian, or the executor of his or her estate provides appropriate authorization.
How to Request Your Jamaica Hospital Medical Records
Online:
We have made it easy to request your medical records online. You will be asked to verify your information and complete a HIPAA authorization form.

Medical Records Office:
To request your medical records in person, you’ll need to print and complete all fields on our health information release authorization form, and sign where indicated.
How to Obtain Your Child’s Birth Certificate
We don’t send out birth certificates from our hospital. Instead, New York’s health department handles this task. Therefore, if you’re trying to get a copy of your newborn baby’s birth certificate for the first time, you should contact (212) 788-4520.
Meanwhile, if you simply want an extra copy of your or your child’s birth certificate, or you need a replacement copy after losing your first one, you can ask for one online at
