Fellowship Program Palliative Care
Application Information
To be considered for an interview, candidates must satisfy the following minimum requirements:
- Have successfully completed training in an ACGME or AOA -accredited residency program in Anesthesiology, Emergency Medicine, Family Medicine, Internal Medicine, Neurology, Obstetrics and Gynecology, Pediatrics, Physical Medicine and Rehabilitation, Psychiatry, Radiation Oncology or Surgery
Be board-certified or board-eligible in one of the above mentioned specialty areas. - Currently have or are able to obtain a license to practice medicine in New York prior to the date the fellowship is scheduled to begin.
- A recent photograph
- Curriculum Vitae (must be month/year specific)
- A copy of your current state medical license (if applicable)
- A copy of your current DEA certificate
- A copy of your USMLE, Comlex and/or National Board Certificate
- A notarized copy of your Medical Diploma
- Certified medical school transcript
- Copy of all board certification certificates
- A personal statement, which outlines your reason for pursuing a career in palliative medicine, your experiences with palliative care, and your career goals.
- Three letters of reference (One from your current or prior Program Director). These letters should be current and attest to personal qualifications, scholastic achievements, and clinical capabilities as related to palliative care.
For foreign graduates only:
- A copy of your Green Card or proof of U.S. citizenship or visa status (preference will be given to U.S. Citizens or Green Card holders; J-1 is allowed)
- Verify that visa status is current
- A notarized copy of your ECFMG Certificate
Application materials will be submitted to the Program Director of the Palliative Medicine Training Program for review. Interviews will be scheduled for the most competitive candidates
