To join our fellowship, candidates must satisfy the following requirements:

• Have successfully completed training in an ACGME or AOA -accredited residency program in Family Medicine or Internal Medicine, or current PGY-3 in good standing.
• Be board-certified or board-eligible in one of the above-mentioned specialty areas.
• Currently have or are able to obtain a license to practice medicine in New York prior to the date the fellowship is scheduled to begin.
• A recent photograph
• Curriculum Vitae (must be month/year specific)
• A copy of your current state medical license (if applicable)
• A copy of your current DEA certificate
• A copy of your USMLE, Comlex and/or National Board Certificate
• A notarized copy of your Medical Diploma
• Certified medical school transcript
• Copy of all board certification certificates
• Completed application
• Three letters of reference (One from your current or prior Program Director). These letters should be current and attest to personal qualifications, scholastic achievements, and clinical capabilities as related to Integrative Medicine care.
