The RISE Clinic at Jamaica Hospital offers three primary forms of care: HIV primary care, Hepatitis C Management, and PrEP to prevent HIV. HIV Primary Care

Persons living with HIV can find a medical home at RISE clinic. Here, we provide full spectrum primary care services incorporated into your routine HIV treatment. This means that your basic health care needs are addressed alongside HIV management (see our same-space services below to understand the breadth of our care). Those receiving HIV Primary Care will also be followed on an ongoing basis by a clinical pharmacist and case worker. We will advocate for your health and wellbeing from achieving and retaining undetectable viral load status to living a successful and happy life.

Hepatitis C Management and Cure

Persons living with chronic Hepatitis C can get cured at the RISE Clinic in Queens, New York! Our team provides consultation, management, and treatment of Hepatitis C along a 12-15 week care course. Our management follows the AASLD guidelines in diagnosis and therapy for Hepatitis C. Our team pharmacist and case worker assist in your care.

PrEP to Prevent HIV

The RISE clinic in Queens, New York assists in incorporating Pre-Exposure Prophylaxis to prevent HIV across our larger Family Medicine practice. Provider training and education, consultation access and dedicated navigation services allow RISE to promote PrEP to prevent HIV in our community.

The Rise Clinic is conveniently located at Jamaica Hospital’s Family Medicine Center, only 1 block from the hospital. The Center offers many other clinical services, including:

The RISE Clinic at Jamaica Hospital treats patients at our Family Medicine Center. Care of delivered by a multi-disciplinary team consisting of a family physician, infectious disease pharmacy specialist, licensed social worker and case manager, and a patient navigator.
